INFORMATION AND PLANNING
Congratulations.
For an appointment with a member
of the parish staff who will help you
to arrange the Baptism,
please take a moment to provide us
with your personal information.
After completing the intake below,
we’ll contact you to schedule your visit.
At that time,
we’ll confirm the date of the Baptism.
And know how happy we are for you!

FAQs
NOT MARRIED?
If the parents are not married,
the child's birth certificate is required.
GODPARENTS?
Should you choose two godparents,
one must be male, the other female.
Should you choose only one,
he/she must be a practicing Catholic.
Godparents must be at least 16
and have received the Sacraments of
Baptism, Communion and Confirmation.
If the godparent is married,
he/she must be married in the Church.
A non-Catholic can be a "Christian Witness"
but he/she must be baptized.
Please have each Sponsor complete and
sign the following witness form.
The Sponsors must physically
sign the form to produce a
pen-on-paper signature.
We cannot accept computer
generated signatures on this form.
You may bring this form with you to
the meeting or send it in
to the Parish Office.
THE DATE?
The date of the Baptism can be confirmed
only after meeting with the Parish Staff.
THE MEETING?
Your visit to the office is simply
an opportunity for us to share conversation,
to speak with you.
